The Chronic Conditions Management program is available to those in the community who require an exercise based rehabilitation program for lung and cardiac conditions. The Cardiac and Pulmonary Rehabilitation groups are run under “best practice” guidelines set down by the National Heart Foundation and Australian Lung Foundation. They provide education and exercise sessions for patients with pulmonary disease and for Outpatient (Phase 2) and Maintenance (Phase 3) patients. All patients access this program will be assessed by a physiotherapist or exercise physiologist prior to entry into the group programs.
